Volunteer Application Form

Dear Prospective Volunteer:

Thank you for your interest in volunteering with Prevent Blindness Illinois (PBIL) and the Illinois Society for the Prevention of Blindness (ISPB). Our vital work and success is made possible by our staff and our current and future VOLUNTEERS like you! There are many volunteer opportunities available for both individuals and groups that help us achieve our mission of preventing blindness and preserving sight. If you are interested in becoming a volunteer, please take a few minutes to complete the Volunteer Application form.

Once we review your application, we will contact you in order to discuss your volunteer interests and availability. If we move forward with your application, we will ask you to complete a Volunteer Packet, which will include your consent to criminal background check. It is the policy of Prevent Blindness Illinois and ISPB to conduct criminal background checks on all persons seeking consideration for volunteer positions.
